By completing this form and requesting an AUTHORIZED FACTORY SERVICE CENTER TECHNICIAN to perform HEAT SIPHON FIELD SERVICE, YOU AGREE to be responsible for all service center charges, which the Service Center will specify to you AT THE TIME YOUR SERVICE CALL IS SCHEDULED if the problem reported below by you IS NOT COVERED BY YOUR HEAT SIPHON FACTORY WARRANTY.

IN ORDER TO DETERMINE WARRANTY AND PAYMENT RESPONSIBILITY YOU MUST PROVIDE A VALID SERIAL NUMBER AND/OR PROOF OF PURCHASE RECEIPT. In any case, there is a non-refundable trip rcharge of $65 for the initial visit per service authorization.
